Whenever I see a movie (Fast Five) or a TV show (Sons of Anarchy) or in a video game (Hitman: Blood Money) show a cocaine basement or garage, it has women in underwear cutting the cocaine. Why do they have to always be in underwear and why is it always women?

Probably so they don’t steal the coke!
1: it’s predominantly men who watch/play those, so the makers appeal to their audience.
2: they’re showing that the cocaine is so good they have tons of women who are addicted to it and therefore work to keep it going.
Because it is less endearing to the viewer if it is burly, unwashed Russians cutting the cocaine, like in real life.
They’re derivative of New Jack City.
See the shows you mention aren’t very original, so they steal scenes. Especially scenes that allow them to show a little flesh.
3 reasons –
1) The less clothing you wear, the less places there are to steal product to sell on your own.
2) A bit of fan service – if a movie (and especially video game) has a scene that has people cutting cocaine, it is probably a movie that has been marketed to young men. Hence, showing young women will attract a bigger market.
3) Censors are probably the reason they are in underwear. If they were all completely naked, they may have to change the rating, which would limit the available market.
Ether is flammable so clothing is not advisable in cutting cocaine from brick to powder as it might catch on fire as the ether splashes on the garments. Masks and panties are bad enough. As for why they do not use men, that has mainly to do with the relative size of men to women and the fact that most men are on guard. It also makes sense not to be wearing clothing because it is very hot under the lights and it is very likely the clothes would get you arrested if the cocaine was on it.
